Make Insurance Simple LLC (“we” or “us”) is an insurance brokerage. This policy explains what information we collect, why we collect it, who we share it with, and the choices you have. It covers makeinsurancesimple.com, our intake and enrollment forms, and information you give us by phone or email.
1. Information we collect
We collect information you give us when you ask for a quote, apply for coverage, or contact us:
- Contact details: name, email address, phone number, and ZIP code.
- Personal details: date of birth, and information about the household members or dependents you want covered.
- Health details: medications, overnight hospital stays, the doctors you want to keep, and your current coverage.
- Financial details: your monthly insurance budget and, if you choose to share it, your household income.
- Enrollment details: the additional information an insurance company or the Health Insurance Marketplace requires to enroll you.
- Business details: if you ask for a group quote, information about your business and the people you want to cover.
Our website and the tools it uses also collect basic technical information automatically, such as your browser type, device, and the pages you visit.
2. How we use your information
- Compare plans and prepare your quote.
- Help you apply for and enroll in coverage.
- Service your coverage after you enroll.
- Contact you about your quote, application, or coverage.
- Meet our legal, licensing, and insurance company obligations.
We use your information only for these purposes unless you give us permission for something else. You can withdraw that permission at any time.
3. Who we share it with
- Insurance companies whose plans we quote for you or enroll you in.
- The Health Insurance Marketplace and the approved enrollment platforms we use to submit applications.
- Licensed agents and partner agencies that work with us on your quote or enrollment.
- Service providers that run our forms, email, client records, and website. They may use your information only to do that work for us.
- Government agencies, regulators, or courts when the law requires it.
We do not sell your personal information. We do not share it with other companies for their own marketing.
4. Health and financial information
We share your personal health and financial information only to quote, enroll, and service your insurance, or as the law permits or requires.
5. Marketplace privacy notice
This section applies when we help you with coverage through the Health Insurance Marketplace.
- Our authority to collect it: We are agents and brokers registered with the Marketplace. We collect your information under our agreements with CMS, the federal agency that runs the Marketplace, and federal rules at 45 C.F.R. §§ 155.220 and 155.260.
- Why we collect it: To help you apply for coverage and financial help, enroll in a plan, and service that plan.
- Who may receive it: The Marketplace, the enrollment platform we submit your application through, and the insurance company you choose.
- How it may be used: Only for the purposes listed in this policy, unless you consent to another use.
- Whether you must provide it: Giving us this information is voluntary.
- If you choose not to provide it: We may not be able to give you an accurate quote, check whether you qualify for savings, or enroll you.
Before we help you apply for Marketplace coverage or financial help, we ask for your consent. You can withdraw it at any time by emailing help@makeinsurancesimple.com.
6. How we contact you
When you give us your phone number or email address, we may call, email, or text you about your quote, application, or coverage. To stop texts, reply STOP. To stop calls or emails, tell us by phone or email.
7. Seeing, correcting, or deleting your information
You can ask to see the information we hold about you, or ask us to correct or delete it. Call 813-395-9080 or email help@makeinsurancesimple.com. We answer correction and deletion requests within 10 business days. We may need to keep some records when the law or an insurance company requires it.
8. Cookies and outside tools
Our website uses cookies and similar tools to make the site work, to understand how visitors use it, and to measure our advertising. Some features are provided by other companies, including our intake form (Jotform) and the Google reviews shown on our site. Those companies, and any analytics or advertising services we use, may collect information about your visits to this and other websites under their own privacy policies. Our site does not respond to browser “Do Not Track” signals.
9. How we protect and keep your information
We limit access to the people who need your information to help you, and we use safeguards designed to protect it. No system can be guaranteed completely secure. We keep your information for as long as we service your coverage and for the record-keeping periods that insurance laws, insurance companies, and the Marketplace require. Marketplace rules require us to keep a record of your consent for at least 10 years.
10. Children
Our website is not directed to children under 13, and we do not knowingly collect information from them. A parent or guardian may give us information about a child they want covered.
11. Changes to this policy
We post any changes on this page and update the effective date. We review this policy at least once a year.
